Senior/Lead Pastor Membership Application

Before beginning the application process please note the following:

This application form is for the Senior/Lead Pastors applying for MFI membership. If you are a support leader, your senior/lead pastor must first be a member, and once he/she is, he can request membership for any support leader by logging in as a member and filling out the Support Leader Membership Application under "Join."
To apply for membership we request that you provide the name, phone number and email address of a current senior/lead pastor member of MFI with whom you have a relationship and who will serve as your sponsor. Please provide that information where requested below and ask your sponsor to log in and fill out a sponsor form.
In the questionnaire portion of this application the full version of each question can be found by placing your cursor over the underlined portion of the field.
Please note that in order to submit this form you will need to use the designated area below to upload the following:
- An organizational chart that reflects the functional structure of your church.
- Your church's articles of incorporation.
- Your church's constitution/by-laws.
